In healthcare, timely communication is everything. When patients are admitted, discharged, or transferred, care managers need to know immediately to provide the best support. That’s where ADT alerts come in, and Sevida is taking this process to the next level. In this blog, we’ll explore what ADTs are, their history, and how Sevida supports care managers in achieving peak performance.

What Are ADTs?

ADT stands for Admission, Discharge, and Transfer—key moments in a patient’s care journey. ADTs ensure care teams are informed when patients move between care settings, like when a patient is admitted to the hospital, transferred to another facility, or discharged home. These alerts are critical because they give care managers a chance to step in and support patients when they need it most.

A Brief History of ADTs

ADT alerts have been around for decades, originally as part of hospital systems. They were designed to improve communication between hospitals and providers. In the past, this often involved faxes, phone calls, or manual record updates— slow and error-prone processes. Over time, as healthcare systems became more connected and ADTs evolved into electronic notifications. Even so, many platforms still struggle to make this data actionable for care teams.

How Sevida Makes ADTs More Effective

Sevida takes ADT alerts to the next level by incorporating them into a configurable workflow. On the Sevida Dashboard, ADT alerts are displayed in the care manager’s list of Alerts, so they are impossible to miss.

Here’s how it works:

  1. Instant Alerts: When a patient is admitted, discharged, or transferred, Sevida immediately flags this change on the care manager’s dashboard and in the patient’s profile. Alert notifications can also be sent by email and/or text message.
  2. Tasks: A task is automatically created for the care manager, prompting them to follow up with the patient within a set time frame—such as within 4 hours. If multiple high-priority ADTs tasks arise simultaneously, an AI algorithm steps in to assess their urgency and calculates a priority score for each task. Care managers can then complete the most critical ADT tasks first.
  3. Reminders: Care managers receive a notification one hour before the ADT task is due, ensuring that every follow-up is completed on time.
  4. Analytics: Organizations can gain valuable insights into their ADT workflow tasks through a comprehensive analytics dashboard. This tool offers a clear view of key metrics, such as the average time to complete ADT follow-ups, enabling supervisors to generate detailed reports and pinpoint opportunities for improvement.

The ADT workflow keeps care managers focused on what matters most: timely and effective patient care. By automating the follow-up process, Sevida ensures that no alert goes unnoticed and no patient is left waiting.

Why This Matters

ADT alerts in Sevida empower care managers to act quickly, which can make all the difference in a patient’s recovery. For example, if a patient is discharged from the hospital, the care manager can follow up to ensure they understand their medications, have transportation to follow-up appointments, or feel supported in their recovery plan. These small steps can prevent readmissions and improve overall outcomes.

Sevida doesn’t just deliver information—it turns it into action. With the ADT alert feature, Sevida is proving that the right tools can transform how we support patients. By making critical information clear, actionable, and timely, Sevida is helping care teams deliver better care and build stronger patient relationships.
